I am logged in and have no problems navigating he MCA Website.
I need a copy of the MCA Event Insurance Application form and can not find it anywhere in the MCA website. If any one can help, send directions or copy of form to address below.

In the carlite 65 response above is a link that will get you there. It's number 12-2 on the list.
Tom, If you use the club tool box link all you get is a an e-mail link to the MCA Office manager. Where this form is buried on the club web site is beyond my ability to find.
There should ony be one storage location for this file. If it ever changes you don't want many different copies or versions of the form. There should be a direct link in the Club Tool Box, hint hint.
I am looking for the form again and can't seem to find it on the website.
Can someone provide a link or the MCA office send it to me?

On the home page under the red Sections Folder, Related Links, Heacock Classic. The main page will say PAGE NOT FOUND, but on the right side is a link to show insurance form.

In preparation for our regional clubs annual show next April I am once again requesting the liability insurance form(s) needed for our show. The link to Heacock Insurance leads me to an insurance application that requires a fee of $1.00 per car ($150.00 minimum). Isn't this fee waived for regional MCA clubs? If so, I need an application that does not require this fee.
One other item on the Heacock application is the question regarding the selling of food and beverage. We use an independent food service provider. The "club" does not sell food or beverage. On the application form, how should this question be answered?
